How can payments be made?
Payments are due at the time of service. We accept Cash, Visa, MasterCard and Debit Cards. We Accept Checks!

What insurance providers do you accept?
We are in the panel of a number of health insurance plans including: Medicare, Medicaid, SUMMA Care, Medical Mutual, Aetna, United Health Care, Buckeye, Unison, and Caresource. Please call the office for more information regarding health insurance.

How can I get my prescription refilled?
Please call during office hours for medication refills. Each request is reviewed by Dr. Amanambu. If your last appointment with our office has exceeded 3 months, a follow up appointment is required before a refill will be authorized. Please call at least 0ne (1) week before your medications run out to ensure that you have your medications in a timely fashion. We also advise that you use one pharmacy for all your medication needs.
